ADA2013:同濟(jì)大學(xué)醫(yī)學(xué)院曲伸發(fā)現(xiàn)25羥維生素D可改善血糖控制

為了評(píng)估在中國(guó)2型糖尿病患者中,25羥維生素D[25(OH)D]的狀態(tài),以及其與葡萄糖代謝的關(guān)系。來(lái)自同濟(jì)大學(xué)醫(yī)學(xué)院的曲伸教授及其團(tuán)隊(duì)進(jìn)行了一項(xiàng)研究,該研究發(fā)現(xiàn)維生素D缺乏在中國(guó)糖尿病人群中非常普遍。25(OH)D水平與血糖和穩(wěn)態(tài)模型評(píng)估的胰島素抵抗(HOMA–IR)呈負(fù)相關(guān),暗示25(OH)D可以通過(guò)改善胰島素抵抗降低血糖。
該研究中,在一個(gè)來(lái)自中國(guó)上海、被診斷為2型糖尿病的患者的橫斷面樣本(592例男性和543例女性,年齡30–80歲)中測(cè)量血漿25(OH)D。測(cè)量空腹血糖、空腹胰島素、糖化血紅蛋白(HbA1c)。根據(jù)上述參數(shù)計(jì)算穩(wěn)態(tài)模型評(píng)估的β細(xì)胞功能(HOMA–β)和HOMA–IR。
該研究結(jié)果表明,血漿25(OH)D的幾何均數(shù)為52nmol/l,且維生素D缺乏[25(OH)D<50nmol/l]和維生素D不足[50nmol/l≤25(OH)D<75nmol/l]的比例分別為53.1%和25.8%。25(OH)D與HbA1c、空腹血糖和HOMA-IR呈顯著負(fù)相關(guān)(r=-0.066,p<0.05; r=-0.122,p<0.01; r=-0.091,p<0.01)。
該研究發(fā)現(xiàn),維生素D缺乏在中國(guó)糖尿病人群中非常普遍。25(OH)D水平與血糖和HOMA-IR呈負(fù)相關(guān),暗示25(OH)D可以通過(guò)改善胰島素抵抗降低血糖。需要前瞻性隨機(jī)臨床試驗(yàn)確定25(OH)D在調(diào)節(jié)葡萄糖代謝中的作用。

To evaluate the status of 25-hydroxyvitamin D [25(OH)D] and its association with glucose metabolism in the type two diabetes Chinese patients.
Plasma 25(OH)D was measured in a cross-sectional sample of 592 men and 543 women aged 30-80 years from Shanghai Chinese patients who have been diagnosed with type two diabetes mellitus. Fasting plasma glucose insulin A1C were measured. Homeostasis model assessment of β cell function (HOMA-β) and homeostasis model assessment of insulin resistance (HOMA-IR) were calculated based on the above parameters.
The geometric mean of plasma 25(OH)D was 52 nmol/l and percentages of vitamin D deficiency [25(OH)D <50 nmol/l] and insufficiency [50 ≤ 25(OH)D <75 nmol/l] were 53.1% and 25.8% respectively. Significant inverse associations existed between 25(OH)D with A1C and fasting plasma glucose and HOMA-IR(r=-0.066 p<0.05; r=-0.122 p<0.01; r=-0.091 p<0.01).
Vitamin D deficiency is common in diabetic Chinese population. 25(OH)D level is significantly negatively associated with plasma glucose and HOMA-IR implying 25(OH)D might lower glucose through improve insulin resistance . Prospective randomized clinical trials are warranted to determine the role of 25(OH)D in regulating glucose metabolism.
